This is a reminder that next month, May 2022, is a client survey month! Please administer the outcome slips/ surveys with each individual who receives diapers from your agency in the month of May.
As a reminder, we ask for these outcome slips to be completed at the following times:
- When a client gets diapers from a partner for the first time.
- Twice annually from all clients getting diapers in the months surveys are requested by SCDB (May and November 2022).
You can print them from the partner manual, where they are available in English, Spanish, and Nepali. As usual, you can turn in these slips at your pickup time.
